Please download, print, complete and sign this booking form and send it with a deposit of £50 per person or full payment if the booking is made within eight weeks of departure.

Surfers Names and Declaration
The surfers names and declaration is to be completed by all holiday members before they will be allowed to participate on any trip. For group bookings, signatures of all members must be completed. If the holiday member is under 18 years old consent must be given and the form signed by a parent or guardian.
